Payment Method & Authorization *Required

Tuition bills automatically every Friday for the following week. To avoid late fees and payment gaps, we require a payment method and authorization on file before your child can start.
In our live system this connects directly to our payment processor’s secure form — we never store full card or account numbers ourselves.
I authorize Core After-School Academy to automatically charge the payment method above every Friday for the following week’s tuition and any enrolled extra programs, for as long as my child remains enrolled. I understand that if this payment method fails or is not kept current, late fees may apply and continued enrollment may be affected, per the Tuition Payment Policy.

Liability Waiver

In consideration for being permitted to utilize the facilities, services, and programs of Core Fitness & Martial Arts, Inc for any purpose, including but not limited to observation or use of facilities or equipment, or participation in any program affiliated with Core Fitness & Martial Arts, Inc, without respect to location, the undersigned, for himself or herself and any personal representatives, heirs, and next of kin, hereby acknowledges, agrees and represents that he or she has, or immediately upon entering or participating will inspect and carefully consider such premises and facilities or the affiliated program. It is further warranted that such entry to Core Fitness & Martial Arts, Inc. for observation or use of any facilities or equipment or participation in such affiliated program constitutes an acknowledgement that such premises and all facilities and equipment thereon and such affiliated programs have been inspected and carefully considered and that the undersigned finds and accepts same as being safe and reasonably suited for the purpose of such observation, use, or participation.

IN FURTHER CONSIDERATION OF BEING PERMITTED TO ENTER CORE FITNESS & MARTIAL ARTS, INC. FOR ANY PURPOSE, INCLUDING BUT NOT LIMITED TO OBSERVATION OR USE OF FACILITIES OR EQUIPMENT, OR PARTICIPATION IN ANY PROGRAM AFFILIATED WITH CORE FITNESS & MARTIAL ARTS, INC., THE UNDERSIGNED HEREBY AGREES TO THE FOLLOWING:

1. THE UNDERSIGNED HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE the Core Fitness & Martial Arts, Inc its directors, officers, employees, and agents (hereinafter referred to as "releasees") from all liability to the undersigned, his personal representatives, assigns, heirs, and next of kin for any loss or damage, and any claim or demands therefor on account of injury to the person or property or resulting in death of the undersigned, whether caused by the negligence of the releasees or otherwise while the undersigned is in, upon, or about the premises or any facilities or equipment therein, or participating in any program affiliated with the Core Fitness & Martial Arts, Inc.

2. THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any loss, liability, damage, or cost they may incur due to the presence of the undersigned in, upon, or about the Core Fitness & Martial Arts, Inc premises or in any way observing or using any facilities or equipment of Core Fitness & Martial Arts, Inc or participating in any program affiliated with Core Fitness & Martial Arts, Inc whether caused by the negligence of the releasees or otherwise.

3. THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH, OR PROPERTY DAMAGE due to negligence of releasees or otherwise while in, about, or upon the premises of Core Fitness & Martial Arts, Inc and/or while using the premises or any facilities or equipment thereon or participating in any program affiliated with Core Fitness & Martial Arts, Inc.

4. THE UNDERSIGNED HEREBY AGREES TO SUPERVISE AND BE RESPONSIBLE FOR MY MINOR GUESTS. THE UNDERSIGNED further expressly agrees that the forgoing RELEASE, WAIVER AND INDEMNITY AGREEMENT is intended to be as broad and inclusive as is permitted by the law of the State of North Carolina and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

I authorize Core Fitness & Martial Arts, Inc to use photos, and or other likeness of myself and/or my child or the child for whom I have legal guardianship, for any promotional materials regarding Core Fitness & Martial Arts, Inc programs, facilities or services. Such likeness will not be sold to other parties. Promotional materials bearing these likenesses may be distributed for free to the public and posted on the Core Fitness & Martial Arts, Inc website. Core Fitness & Martial Arts, Inc reserves the right to use any photo or likeness for a time period beginning when this form is signed and ending upon written request of participant, parent or legal guardian.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Core Fitness & Martial Arts, Inc has put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending could increase your risk and your child(ren)'s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Core Fitness & Martial Arts, Inc may result from the actions, omissions, or negligence of myself and others, including, but not limited to participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at Core Fitness & Martial Arts, Inc or participation in the attractions ("Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Core Fitness & Martial Arts, Inc, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Core Fitness & Martial Arts, Inc, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any activity at Core Fitness & Martial Arts, Inc.

I HAVE FULLY READ AND UNDERSTAND THE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AND I AM AWARE THAT BY SIGNING THIS AGREEMENT I MAY BE WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. I RELEASE CORE FITNESS & MARTIAL ARTS, INC. FROM ANY LIABILITY AND FULLY UNDERSTAND ALL RIGHTS ARE GIVEN UP BY SIGNING BELOW AND SIGNING IT VOLUNTARILY ON MY OWN FREE WILL.

Policies & Procedures

Please read each policy below and initial to confirm you’ve read and accept it.

Operating Hours / Registration / Check-Out

  • After-School Academy operating hours are Monday – Friday from school pick-up – 6:00pm.
  • Summer Camp operating hours are Monday – Friday from 7:00am – 6:00pm.
  • Prospective students and parents are encouraged to schedule a tour of our facility prior to enrollment.
  • Registration must be completed and approved prior to a child attending the program.
  • Parents/Guardians are responsible for completing and updating all sections of the registration.
  • Upon enrollment, a non-refundable registration fee of $50 ($40 each for 2 or more children) is due.
  • All-Day Care will be offered for Teacher workdays, holidays, and inclement weather days from 7:00am – 6:00pm; we may need to open late and/or close early on inclement weather days and will notify parents via group text regarding any delays or closings. All-Day Care days, with the exception of inclement weather days, require pre-registration, and any cancellations must be given no later than 24 hours in advance in order not to be charged for the day. The All-Day Care fee is $20 per child in addition to regular tuition paid for the day; the All-Day Care fee for non-After-School students is $40 per day.
  • Check-Out Procedure: Authorized pick-up individuals (these individuals must be listed on the registration) will check out their child in person at pickup; anyone picking up a student will need to physically come into the lobby to check out their child(ren).

Release of Liability

I give permission for my child/children to attend Core Fitness & Martial Arts, Inc./Core After-School Academy. My child may also participate in any/all of the activities provided by the program. I surrender my rights to litigation, civil or criminal, to any volunteer or employee of Core Fitness & Martial Arts, Inc. for injuries, whether physical or emotional, which may occur during normal activities, as the law does allow.

Illness / Injury / Medication Policy

  • Illness Policy: If a child becomes ill while at the Academy (including but not limited to fever, vomiting, rash, lice, etc.), parents/emergency contacts will be contacted immediately and expected to pick up their child as soon as possible; ill children are immediately separated from the group and supervised by a staff member until they are picked up. If your child is showing signs of illness at home (including fever at or above 100, vomiting, or lethargy), please do not send them to the Academy. If a child presents with a fever of 100 or above at the Academy, they cannot return until 24 hours after the fever has passed. Any child whose physical condition appears to be unsafe to other children and staff will not be permitted to stay.
  • Injury Policy: Any/all injuries will be reported to parents via text (non-serious) or a call (serious) from staff, and an injury report may be required to be signed by a parent/guardian.
  • Administration of Medication Policy: If a child is required to take medication while at the Academy, a parent must complete a medical consent form along with providing the medication; any medications must be in the original container, labeled with the child's name, physician information, specific dosage instructions, and the time the medication is to be administered.

Withdrawal Policy

When/if a parent decides to withdraw their child from the program, a withdrawal form must be completed and the required 2 weeks' notice given, along with tuition being paid for the 2 weeks.

Personal Items Policy

Personal Belongings: Each child will have a cubby in which to store their personal belongings such as backpacks, lunchboxes, karate uniforms, etc. It is important that the child take home all belongings at the end of each day. Core Fitness & Martial Arts, Inc./Core After-School Academy will not be responsible for anything lost, stolen, or damaged while on the premises.

Picture / Video / Social Media Permission

Core Fitness & Martial Arts, Inc./Core After-School Academy has my permission/consent to use photographs and/or video taken at Core Fitness & Martial Arts for all advertising purposes.

Discipline Policy

Discipline Management Policy: We set forth clear and concise expectations/guidelines for our students' behavior and continuously remind our students of those expectations and guidelines. We believe it is important for children to learn, through adult guidance, to make the right decisions as well as to be taught how to handle consequences. Children will always be supervised in all activities and any inappropriate behavior will be handled immediately.

Steps to handling a behavior issue (may include any/all listed, but may not be limited to, depending on the situation):

  • The child will be given a warning as well as an acceptable alternative to handling the situation.
  • The child will be pulled aside privately for discussion with a staff member.
  • Depending on the specific incident, the child will be given a consequence directly related to the situation, with staff communicating the reasons behind it.
  • If necessary, a discipline form will be completed for the parent to read and sign that evening at pick-up (suspension or dismissal from the program may occur, depending on the issue, at the discretion of the owners).

Termination of Enrollment Policy

  • If the rules and policies of our program are not followed, either by a student or parent, we reserve the right to terminate enrollment at any time, as deemed necessary.
  • In such an event, 2 weeks of tuition due for withdrawal will be due, and no refunds of tuition paid in advance will be given.
  • Enrollment will be terminated immediately for any of the following, including but not limited to: failure to comply with agreed-upon policies; destructive or harmful behavior that persists even with parent cooperation; non-payment of tuition or late fees, or recurring late payment; repeated failure to pick up the child at scheduled times; inability of our program to meet the child's needs without additional staff; continuous bowel accidents and inability to use the restroom independently; blatant disrespect toward staff, self, others, or property; or false or undisclosed information pertinent to the care of the child.

Transportation Policy

  • Core Fitness & Martial Arts, Inc./Core After-School Academy has my permission/consent to transport my child to and from their school and on field trips.
  • Once my child is dismissed from school, they are to go directly to the van/bus pickup location.
  • Failure to get to the van/bus on time may result in the child being left at school; if a child misses the van/bus, the parent will be contacted by the school and will then be responsible for picking their child up.
  • We are on a tight afternoon school pick-up schedule and will not be able to return to a school to pick up a child.
  • Parents are requested to communicate to us if their child will not need to be picked up at school on a particular day.

Tuition Payment Policy

  • Registration Fee: $50 ($40 each for 2 or more children) per child, due upon enrollment.
  • Late Payment Fee: $10 per child, automatically assessed if the week's tuition is not paid before the Monday of the week attending.
  • Late Pick-up Fee: $1 per minute late, automatically assessed if a child is checked out past 6:00pm (or past 6:30pm for Extended Care students).
  • Tuition will be processed through mandatory automated billing on a weekly basis for the duration of the school year.
  • Parents will need to provide a credit/debit card or voided check.
  • Autopay is required to be set up by parents (or Core staff on the parent's behalf) to continue enrollment.

Program Selection

Please select the program for which you are enrolling: Full-Time After-School Care (4–5 days/week) — $75 1st Child, $65 2nd Child, $55 3rd Child, or Part-Time After-School Care (1–3 days) — $60 1st Child, $55 2nd Child, $50 3rd Child. Yearly tuition is $3,075 (Full-Time) or $2,475 (Part-Time), payable in full or weekly over 41 consecutive weeks.
